The following changes to policy information require the policyholder to complete a Change Form. Each type of change has its own specific form.

Download Policy Change Forms

Name Change – Download the name change form.

Change Beneficiary – Download the form to change a beneficiary on a policy.

Delete Person from Policy – Download the form to DELETE a person from a policy.

Add a Person to a Policy – Please call Aflac toll free at 800.992.3522 to add a person to an existing policy.

The above Change Forms must be completed by the policyholder wishing to make the change. For more information, check out the FAQ.
